‘The LSCB Independent Chair should, within six months, instigate a multi agency audit of good practice in seeking the wishes and feelings of children with disabilities resulting in a package of up to date, well informed professional guidance, procedures and training within twelve months of the audit’

1) The LSCB to require the ‘Disability Children’s Planning Group’ to audit good practice when seeking the wishes of children with disabilities.

2) The DCPG to identify professional guidance with respect to good practice when seeking the wishes & feelings of children with disabilities

The Chief Executives/Officers within education and health should, within six months, work together in developing agency protocols that address how the needs of children with disabilities are assessed and met on the rare occasions when the children do not attend.


- Health Trust’s to utilise work from IMR recommendation around streamlining appointment processes for children with complex neuro-disability

- Health Trust leads to develop a robust communication system between education and themselves to identify children missing education/ health appointments

- Education Leeds, mainstream schools and and Silcs work together to establish a monitoring framework and a procedure for remedial action for children who are not accessing services.

6.2.2


The Chief Executives of the hospital and community trusts should, within six months, review and amend accordingly, guidance concerning documenting in professional records the rationale underpinning decision making with respect to requesting post mortem examination. This should be followed by thorough dissemination and training to all relevant health staff.


- LTHT to undertake a review of the guidance issued to medical staff around the requesting of Post Mortems (PM)

- outcome of the review to ensure medical staff document their decision to request a PM or not

- process to audit records where PM’s may have been required /requested
- SUDIC process to consider and comment on PM requests

Chief Executives within NHS Trusts should, with immediate effect, ensure that the correct procedures for making a referral to CYPSC are emphasised in training for medical staff of all levels of seniority

Chief Executives within NHS Trusts should, within six months, review current legal procedures regarding intervention in the best interest of the child when faced with refusal of parental consent and raise staff awareness throughout their respective organisations.
