STATEMENT from Independent Chair LSCB
The death of this young person was a tragedy for her family and our sympathies are with them. We convened a multi agency Serious Case Review to see whether there were things we could learn about how we worked together to support her and her family.
The Leeds safeguarding children Board and its partners have accepted the review findings in full. There was good practice and there were also some areas where we could have done better. A multi agency action plan has been drawn up. This plan makes clear what we need to do to improve how we offer support in the future. Implementation of the action plan will be monitored by the LSCB Performance Management sub committee.
