“This was a deeply upsetting and tragic case for the family and the local community.
It is only right that the Leeds Safeguarding Children Board (LSCB) commissioned a thorough, detailed and independent review of the circumstances of Child J’s death to identify what lessons could be learned, even though the person responsible has been brought to justice.
The findings have been accepted and several recommendations were made, all of which have been acted upon.
The LSCB is monitoring the impact of these on practice in Leeds.
Work is ongoing to improve communication between all organisations responsible for child protection in Leeds, all of whom remain totally committed to the protection of vulnerable children and young people.
Agencies are working together to do what they can to prevent something similar ever happening again.”
