Application for Training

Please Note: This application form is only applicable to key constituent agencies of LSCB or voluntary agencies based in Leeds.  The form can be used for all LSCB courses where no fee is payable.

To Apply: Complete and submit this form.  You will be informed by mail approximately 1 month prior to the course whether your application has been successful on this occasion.  One applicant only per application form.

Non Attendance: Please note that failure to attend without apologies will result in your organisation being invoiced £50 to cover administrative costs.

Please let us know if you have any special requirements in relation to your participation on the course (eg mobility difficulties, if you require sign language interpretation, or prefer large print handouts etc) and we will endeavour to meet them.
 
Managerial Support
You are required to obtain the support of your line manager for your training application.  Your line manager must agree to release you from your post to attend the course and discuss this training and learning within supervision.  It is your responsibility to ensure your line manager is in agreement with undertaking these requirements before you submit your application form.  Application forms without the manager's support will not be considered.
